## Review and follow-up

| Question | Number assigned | Number responding | Evidence observed | Teaching response | Fresh check and date | Next decision |
| --- | --- | --- | --- | --- | --- | --- |
| | | | | | | |

Record counts alongside percentages. A blank response can reflect time, access
or instructions; do not automatically classify it as lack of understanding.
Keep names and student identifiers out of publicly shared copies.

## Worked example, not real student data

Question: solve 5x + 2 = 32, explaining and checking your steps.
Answer: subtract 2, divide by 5; x = 6. Check: 5 × 6 + 2 = 32.
Possible evidence: the learner subtracts the coefficient instead of dividing.
Response: revisit multiplication and equality-preserving operations.
Fresh check: solve 3x + 5 = 26; x = 7. Explain the operations.
Next decision: choose support after inspecting reasoning, not the score alone.
